If you want to withdraw from an online KALYIX or Tip contract, you can submit the request electronically here.

Notes

This form is intended for withdrawal requests relating to contracts with KALYIX or the Tip product area. Please provide enough information so we can identify your request.

A phone number is optional and only used for quick follow-up questions. Processing usually happens by e-mail.
